Trigger "Departure & Program Exit" modal when deactivating a client

Use this process when a client has exited your program. Deactivating a client records their departure details and removes them from your active caseload.

STEPS

1. Navigate to the client’s profile

Search for or open the client you want to deactivate from your caseload or client list.

2. Click “Deactivate”

Click the Deactivate button in the top-right of the client’s profile. The Deactivate Client form will open.

3. Enter the exit date

In the Date Exited Program field, select the date the client left your program.

4. Select the departure status and type

Choose the Departure Status (e.g. Successful) and Departure Type (e.g. Graduation Time) that best describes how the client exited the program.

5. Record employment and legal status at exit

Select Yes or No for Employed at Exit , and select Yes or No for New Charges at Exit.

6. Add departure notes (optional)

Enter any relevant context in the Departure Notes field, such as “Participant has graduated.”

7. Complete employment details

Under Outcome Measures, choose the client’s Employment Status (e.g. Part-time employment) and add any context in Employment Notes.

8. Record housing and education information

Select an option under Housed (Yes, No, or Unknown), choose the client’s Education Level (e.g. GED or alternative credential), and add any Education Notes if applicable.

9. Indicate driver’s license status

Select Yes, No ,or Unknown under Has Driver’s License.

10. Confirm the deactivation

Click Confirm Deactivation at the bottom of the form. The client will be moved out of your active caseload and all exit details will be saved.
