Staff can now send a self-report form to participants to be filled via the Reconnect Community app on their iOS or Android smartphones. This form can be scheduled monthly or sent ad hoc, and staff can view the form entries in Reconnect Manager if filled out within the assigned window.

Schedule a form

Navigate to Clients.
Select a client and click on their name to view their profile.
Click EventsAdd Event.
Set the Start Date, Start Time, End Date, and End Time of the survey.

This date and time range represents the window a participant has to complete the form.

You can set forms to repeat with the following criteria:

Does not repeat (default)
Repeats monthly

There are two available Event Reminders that can be activated by selecting their respective checkboxes.  Set the checkboxes to remind clients of an upcoming form; reminders can be set in increments of minutes or hours.
Event Description is used for more detailed information and is visible to the client.
Select the form you would like to send in Assign a Form to an Event. Currently, only the check-in form is available.
Notes may be used for more detailed information; event notes are not visible to the client, only to staff.

Participants complete form

Tip: Participants must be logged in to the Reconnect Community app (3.8 or greater) to receive and complete the form.

Participants will see the form available to fill on the Reconnect Community app home screen.



Upon touching the Forms button, participants will be prompted to fill out the form.



If the participant has filled out the form before, fields will be pre-populated with prior responses (with the exception of the Contact with Law Enforcement questions).

Once the form is completed and sent, participants will receive a confirmation.



View a completed form

Navigate to Clients.
Select a client and click on their name to view their profile.
Click Forms. You will see a table listing the most recent form completions.
Click on the single eye icon for the selected entry to view.

View all available forms to assign

Navigate to Settings.
Click Forms. You will see a table listing all available forms.
Click on the single eye icon for the selected forms to view.
